A legal name change and special typographic marks can be applied to a diploma, and errors can be corrected.

A student's lived name automatically appears on their diploma. A student can choose to have their legal name appear instead, using the MyUCLA Lived and Legal Names feature.

Legal Name Change

For a legal name to appear on the diploma, a legal name change form and documentation must be submitted to the Registrar’s Office, 1113 Murphy Hall, by the last day of the degree-expected term. There is no fee for this service.

A legal name change submitted after the last day of the degree-expected term must be accompanied by a replacement diploma request. A special-order diploma fee applies.

Special Typographic Marks

Names appear on diplomas in upper-case letters only. For example: MacKay prints as MACKAY; DiCarlo prints as DI CARLO.

To identify name punctuation, accent marks, or spacing on a diploma, use the MyUCLA Diploma Request feature after the third week of the degree-expected term.

Marks that cannot be accommodated on MyUCLA should be entered on a legal name change form and delivered to the Registrar’s Office, 1113 Murphy Hall, by the last day of the degree-expected term. There is no fee for this service.

Diploma Errors

If there is an error on the diploma, return it to the Registrar’s Office, 1113 Murphy Hall, for correction. If the error is the fault of the University, no fee is charged. If the error is the student’s fault, a special-order diploma fee applies.
